The functional and emotional effects of tooth loss

The loss of one or more teeth has a particular impact on the quality of life, affecting not only the functionality and the smile asthetics, but also the physical and mental health.

Dental implants change the way we live: designed as a basic structure to replace a missing tooth, which with the final dental crown prosthetic work looks, feels and function like the natural tooth.

The functional and emotional effects of tooth loss

After the treatment with dental implants, the patient restores the masticatory function and regains the face harmony and the aesthetic of the smile.

What is a dental implant?

The treatment with dental implant represents the most modern method of replacing the missing teeth or that are in an advanced state of deterioration and cannot be saved.

Maintaining the long term integrity and the fact that it takes the functions and the characteristics of the natural tooth, make the dental implant the most recommended solution by the dentists all over the world.

Worldwide, the success rate of dental implant is over 98%, transforming it in a lifetime treatment solution. (According to the US National Library of Medicine National Institutes of Health)

What is a dental implant?

In our country, 7 out of 10 Romanians have at least one missing tooth and 93% of the patients that have chosen dental implants for their oral rehabilitation and satisfied with their choice (accordingly to a market study led by DENT ESTET and the 360insights research agency).

Dr. Cristian Csapo-Martinescu:

"Ceramic dental implants, latest premium solutions in dental implantology, represent the ideal fusion between aesthetics and biocompatibility, with natural adaptation to the biological material of the patient!"

The benefits of dental implant

Improves dental and general health

  • Dental implants prevent bone resorbtion. The natural atrophy of the alveolar bone starts when the tooth is lost, and if it's not acted upon immediately, it brings complications that can led to the loss of the neighboring teeth. Statistically, the bone volume decreases by 50% in the first three years from the dental extraction, which make the following interventions more complex.
  • Dental implants look and work like natural teeth. Once inserted in the bone, these osseointegrate to prevent the loss of bone mass and to maintain the harmony of the face and the health of the soft tissues
  • improves the general health of the body. With strong and functional teeth, patients can return to a healthy and normal diet.

It is a reliable solution

  • Dental implants osseointegrate in 1-3 months from the intervention, so that they fusion with the bone, preventing the risk of mobilizing.
  • With an adequate oral care, dental implants can last for a lifetime, being considered as the smartest investment when talking about replacing missing teeth.
  • The success rate of the treatment with dental implants is over 98% - depending on the implant quality and the case complexity.

Restores the confidence of the smile

  • The complete dental work consisting of the implant and the final prosthesis has a facelifting effect: it corrects the asymmetries of the face and gives the face a rejuvenated appearance (especially in patients with multiple edentations, with an aged facial appearance).

  • It boosts the self esteem and help maintain an optimum oral health state.

  • Your implants are unique. Every new tooth (permanent dental crown) is personalized with the shape, form and dimension similar to the natural teeth in order to bring back the natural look of the smile.

The communication and establishing a trusting relationship between the doctor and the patient are the first steps towards a successful treatment.

Before starting the treatment, the doctor presents to the patient a real overview on his dental health and explains the full process of the design leading to the new smile.

The initial invetigations Iand the complex medical anamnesis of the patient are the mandatory pillons for creating a correct and complete treatment plan.

Moreover, the Patient Care CoordinatorsR  team keeps a continuous open relation with the patients, to offer them the safety and support needed to complete the treatment and for the future maintenance program.

Dr. Alexandru Georgescu:

„We choose Nobel implant for its premium quality, but also for the benefits offered for single implants or for All-on-4 systems. Nobel Biocare dental implant offers more than a smile, it is actually the chance to a new life. It's a choice that offers safety and international validation through scientific studies published in certified jurnals.”

The medical specializations and the vast expertise are the basis criteria for the selection of our dentists in the implantology team.

Our doctors are both the specialists and the artisans with the competence to transform the smile and to improve patients' life. The coordinating doctor takes time to understand the patient's needs, desires and fears and looks for optimum treatment solutions. The personalized care offered has the purpose to build a strong lasting relationship between the doctor and the patient.

The treatments we perform represent the effort of a team of specialists: implantology specialist, prosthetic doctor and dental technician in collaboration with the patient, actively involved in choosing the aesthetics of the new smile. Continuously connected to postgraduate studies and courses, with great experience in complex cases and empathy towards the patient are completely dedicated to the long term health.

The elements of the complete dental implant

The screw / the actual implant: plays the part of the artificial root and is inserted in the bone. This one represents the basis of the tooth and can sustain one or more artificial teeth (crowns). The screw, made of titanium, titanium alloy  with zirconia or ceramics, can be used to replace one tooth when supporting one crown, or can be geared in bridges on implant.

The abutment  connects the screw (implant) with the artificial tooth (dental crown). the abutments are specific to each type of implant and it is extremely important for these to be original and not from other producer, other than the company of the used implants. Poor adaptation, even at a microscopic level, can lead, over time, to imflammation of the peri-implant tissues of rupture of the abutment.

The dental crown represents the aesthetic aspect of the artificial tooth. This is made in the dental laboratory, so that it is as close in shape and color with the natural tooth.

Dental implant vs. other treatment options

Since its launch in 1952 and until now, the dental implant has been perfected so that it has become the first choice of specialists for the replacement of missing teeth.

In addition to the dental implant, there are also temporary treatment solutions, less expensive, but with undesirable implications on the long-term dental health:

  • Dental bridge is the traditional solution that involves replacing the missing tooth with a dental work that is supported on neighboring teeth. The prosthetic work consisting of the 3 crowns involves the grinding of the 2 healthy teeth on which it is fixed.
  • Mobilizable denture is a mobile prosthetic work that rests on the gums to replace missing teeth. This can be done for the entire arch or partially for a few teeth.

This accentuates bone resorption, and then an operation with dental implants becomes a complex case.

Below you can find a table comparing the 3 types of treatments for missing teeth:

When is dental implant recommended

If you have lost at least one tooth or if you have a tooth that cannot be saved by using any conservative treatment, the first recommendation of the dentist will be dental implant.

It is very important for you to take the decision on time to prevent bone loss, teeth migration and other negative effects that can influence your oral health, facial look and the quality of life.

When is dental implant recommended

Full mouth dental implants

Patients that need to replace a dental arch or both arches with a new set of teeth can opt for fixed prosthesis systems, with temporary dental work within 24 hours.

Multiple dental implant

In many cases, the patient is missing more teeth, or he can lose them due to the extended degree of deterioration, but the gums and the soft tissues remain healthy. In order to maintain the integrity of the neighboring teeth, the implantology dentist will choose a multiple dental implant solution.

Single dental implant

In case of tooth loss due to an advanced decay, an accident or periodontal disease, that tooth can be replaced with dental implant that looks and feels like a natural tooth.

Dr. Alice Gavrilescu:

„With a modern design, the Bredent implant has an excellent primary stability, fact that make it suitable both for the cases in which we replace one tooth, but also for complex and complete dental arch oral rehabilitation. It is a reliable solution for immediate restauration and implantation, so that the masticatory function can be regained in only one day. ”

Sharon B. - patient from the UK

I’ve had top & bottom implants and now I have a lovely smile! My collaboration with Dr. Dragoș Calangiu was, really from the beginning, very good. He explained everything to me, every stage of what he is going to do, which parts will hurt, which wouldn’t, and really, none of it did. I’d happily do it again if I had to!

Digitally guided dental implant

Digitally guided dental implant is the latest technology in modern dental implantology and implies the insertion of the implant in the bone based on very exact digital measurements.

Digital insertion is a standard in the DENT ESTET clinics and starts with the CBCT digital investigations upon which the surgical guide is elaborated. Besides the accuracy of the implant insertion, digital guided implant procedure offers many benefits for the patients.

Dental implant treatment stages

Planning and the specialized consultation Planning and the specialized consultation

The first step to find out if you are a suitable candidate for dental implants is the medical consultation. Based on the oral examination performed by the doctor in the dental office, the digital radiology investigations (CBCT computed tomography) the coordinating dentist will create the personalized treatment plan that includes the stages of the treatment and the prioritized treatments.

For complete accuracy  and precision during the insertion of the dental implants, a 3D digital simulation will be created based on the CBCT images. Using these results, the doctor will establish the intervention flow. chirurgicală.

Before the intervention, for the safety of the patient, the doctor will require a standard set of investigations. In the particular case that the investigations are not compliant, the doctor has the responsibility to inform the patient and reschedule the intervention.

Cavities, root canal infections and Cariile, infecțiile de canal sau periodontitis are treated before the insertion of the dental implant.

Before the intervention, it is mandatory that the patient to make the routine prophylaxis session, so that the teeth and the soft tissues to be cleaned in a professional manner and the bacteria removed.

Anesthesia and the patient preparation Anesthesia and the patient preparation

Before the patient enters into the dental office to be prepared for the intervention, he will sign the surgery consent (with all the details of the procedure) and the pre-anesthesia questionnaire so that he is correctly informed on the procedure he will take.

In order to offer the patient with a more comfortable setting, we will perform a digital anesthesia with patented technology to control the flow rate and pressure of the anesthesia during the injection. Three speeds are tailored for each injection type to ensure optimal comfort and effectiveness. Furthermore, the implantology doctor will be assisted by the anesthesiologist during the intervention.

In case of complex intervention (sinus lifting, bone addition) it is advised to use the PRGF kit, to create membrane from the patient's blood in the dental office for a faster healing process.

Implant insertion and the final prosthesis Implant insertion and the final prosthesis

In the day of the intervention:

  • In the possible cases, the patient benefits of temporary prosthesis in the same session with the implant insertion.
  • Nobel Biocare, Straumann and Bredent dental implants allow the insertion of the implant immediately after the extraction, decision that can be made only by the implantology doctor, at the extraction time.
  • The case of edentulous patients that receive implant systems like All-on-4, Straumann Pro Arch and Fast & Fixed by Bredent the temporary prosthetic work is made in 24 hours.
  • After the intervention, the patient will receive the medication scheme from the implantology doctor and will perform an OPG (digital panoramic radiography) and CBCT (computed tomography).

 7-10 days from the intervention:

After the dental implant has been successfully inserted into the bone, based on the digital surgical guide made by the implantology doctor, 7-14 days apart from the intervention the sutures are removed - depending on the case complexity.

The final prosthesis takes the specific time depending on the implant type and the complexity of the case:

  • Implants with fast osseointegration like Nobel Active and Straumann SLA ACtive receive the final prosthetic work two months after the intervention.
  • For the complex cases, with laborious interventions with bone addition and sinus lifting, the final prosthesis is performed after 6-9 months from the intervention.
  • For the implant systems such as Nobel All-on-4, Straumann Pro Arch and Fast & Fixed by Bredent the final prosthesis is made after 4-6 months from the intervention.

After the final prosthesis, DENT ESTET is offering to the patient a Quality Assumption Certificate that includes controls and periodic prophylaxis sessions, certifies the quality of the medical act and also offers extended guarantee for the performed treatment.

During this period of time, keeping a good oral hygiene at home, following the recommended periodic dental visits and the prophylaxis sessions is mandatory.

Dr. Dragos Calangiu:

„Fast and Fixed is a treatment method that offers both the functionality and the confidence of the patient. The feeling that the dental arches regain their function and the aesthetics of the real teeth has a "time machine effect" - the patient feels younger. The Bredent experience in the implantology - prosthetic field comes with great outcomes for the patient."

Recovery and recommendations after the dental implant intervention

For the majority of patients with simple interventions the postoperatory discomfort is minimum. These patients have to strictly follow the doctor's recommendations to prevent further complications.

In the case of more complex interventions, patients can feel a local discomfort, swollen gums, bruising, edema and minor bleedings.

The doctor will recommend medication to ease the pain and to calm down the local inflammation and even antibiotic treatment.

To avoid further complications, the patient has to follow the specific recommendations of the doctor:

  • After the intervention the patient has to keep a cold compress on the cheek to reduce inflammation.
  • The patient respects the medication recommended by the doctor.
  • Don't touch with the tongue the area of the intervention to avoid the swelling. The sensitivity will slowly decrease during the recovery period.
  • Brushing can be done at doctor's indications from the first day after the intervention in order to avoid the bacterial plaque accumulation, but it is advised to use a soft bristle brush and in no case the electric brush.
  • Avoid hot food and beverages until the effect of the anesthesia dissapears and avoid coffee or alcohol consumption in the firts seven days after the intervention, due to the risk of bleeding
  • I's not recommended to eat fluory foods because they get sticked on the gums and can lead to inflammation. Instead, soft foods can be eaten until the sutures are removed.
  • Avoid smoking, it can lead to serious complications and to treatment failure.
  • Regulary visits to the dentist are mandatory, as accordingly to the coordinating doctor's recommendations

Caring for your dental implants

Dental implants look, feel like and can be taken care like real teeth.

A dental implant treatment correctly done should not lead to future complications if the patient has a correct oral hygiene ritual, including brushing twice a day, flossing, using mouthwash and oral irrigator.

The prophylaxis and the regular visits to the dentist are, also, mandatory.

Dental implant - contraindications and risks

Risk factors that negatively influence the success of the treatment:

  • Prosthetic overload
  • Local infections
  • Nerve, blood vessels and sinus damage
  • not respecting the after intervention indications

Symptoms of dental implant loss

appear in 2% of the worldwide implant cases:

  • Acute pain and discomfort
  • Implant mobility
  • Gums inflammation
  • Gingival retraction
  • Teeth mobility on implant
  • Purulent secretion
  • Eating difficulty

Prof. Dr. Paulo Malo

“The All-on-4 technology comes as a solution to a great challenge, to oral rehabilitate the edentulous patients, especially those without bone and teeth. So that, after several years of research we discovered that if we insert only 4 implants with a specific angulation and in a certain position, we can create a fixed prosthesis with a high success rate, in many cases of 100%. The success is huge only by taking into consideration the feeling of telling to a patient that has been offered the alternative of a mobile prosthesis, that you can offer him fixed teeth in a day without bone addition and with a success rate higher that in the case of grafting. Furthermore, this new technique that doesn't imply bone addition has lower costs, almost half of the total cost of a complete standard procedure (that includes bone addition) and represents a kind of miracle for the patients."

Frequently asked questions about dental implant:

1

Which type of implant is the best?

The best implant is the one suited for you. Taking into account your needs and desires, the doctor will recommend you a wide array of implants. The most modern and popular among medics and patients are the premium ones. They are backed by tens of years of studies and research, which over time led to the creation of some of the best dental implants, in terms of durability, osteointegration and aesthetics.

2

How many implants are necessary for edentulous patients?

In the case of edentulous patients opting for All-on-4 and All-on-6 implant systems, 10 implants (4 on the mandible and 6 on the maxilla) are needed for a complete smile.

3

Can persons with diabetes have dental implants?

Yes, if the diabetic has stable glycemia and no other medical issues. There are even certain Straumann implants recommended for this type of patients, because they provide better recovery.

4

What kind of anaesthesia is used during the dental implant intervention?

During the intervention, we use a local anaesthesia, but in the case of patients with anxiety, we can administer general anaesthesia.

5

What is the success rate of the dental implant?

On a global scale, the rate of success of the dental implant is 98%.

6

Do I need to take sick leave from work after the dental implant?

No, you don’t need sick leave after the dental implant intervention. You can go back to your daily routine as soon as the intervention is over.

7

Will the dental implant feel like a natural tooth?

Yes, the implant is designed to look and feel like a natural tooth. The osteointegration process is fast, that’s why the body will perceive the implant as it would a natural root.

8

How soon after the procedure can the suture wires be removed?

The suture wires are removed after 7-10 days, but that depends on the complexity of the procedure.

9

How long is the recovery process?

The recovery process lasts somewhere between 4 and 6 months, but it all depends on the genetic material of the patient. The good news is that they can easily go back to their daily activities right after the intervention.

10

How soon after an extraction can I benefit from an implant?

Our doctors recommend a period of 3-4 months of recovery after an extraction. However, there are cases in which an implant can be done right after the extraction. This decision is made by the doctor during the intervention, while keeping in mind the available bone mass and the state of the extracted tooth (if infection is present or not).

11

How long does a dental implant last?

That depends on the type of implant, the genetic material of the patient (he/she has no serious health issues), the medic’s experience and the post-intervention hygiene. A premium dental implant (Straumann, Nobel Biocare) made with the utmost care by the medic can last up to 40 years, if the patient comes every 6 months for routine checks. It’s worth mentioning that oral hygiene plays an essential role in how long the implant will last.

12

How long does the implant procedure last?

The dental implant procedure takes about an hour.

13

What are the chances the body will reject the dental implant?

Correct dental hygiene, regular visits to the dentist (at least twice a year) and cutting down on vices like smoking will ensure the success of the dental implant. The body can’t reject a dental implant, but a lack of dental hygiene will lead to the loss of the implant.

14

Will there be any pain after the dental implant procedure?

The patient won’t feel any pain after the intervention. He can go back to his daily activities without feeling any discomfort.

15

How will I look after the treatment?

17

Am I too old for dental implants?

Age is not an impediment for the dental implant treatment. However, the most qualified person to decide this is the implantology expert. Using a short medical history, he will guide you through each stage of the treatment and will tell you everything you need to know about the intervention.

18

Is the dental implant procedure painful?

Thanks to the innovative sedation techniques found in the Dent Estet clinics, the implant procedure is 100% painless.
